PanCare will be at Walton High School 𝐉𝐮𝐧𝐞 𝟖-𝟏𝟎 to perform sports physicals (and ECG screenings for athletes that haven't received it). Below is a link to the forms that will need to be completed before students get on the bus to see the provider.

As a reminder, forms must be fully completed and parents/guardians must sign at all designated spots. The EL2 form requires parents/guardians and students to sign on pages 2 and 4. If the student comes to the bus and any part of the forms are not filled out correctly or parent/guardian signatures are missing, he/she may be asked to reschedule if there are available appointments.
